Factores pronósticos de los cambios evolutivos de la cardiopatía hipertensiva desde la disfunción diastólica leve hasta la función sistólica deprimida. Revisión sistemática con metaanálisis

Resumen

Introducción: la cardiopatía hipertensiva sobresale dentro las lesiones a órganos blanco por hipertensión arterial, por su la elevada morbilidad y discapacidad.

Objetivo: evaluar la capacidad pronóstica de los diferentes factores asociados a los cambios evolutivos de la cardiopatía hipertensiva hasta la función sistólica deprimida.

Métodos: se realizó una revisión sistemática con metaanálisis de estudios analíticos de casos y testigos, de cohorte y metaanálisis. Para lo cual se realizó una búsqueda electrónica de la literatura para los diversos estudios relacionados con el tema desde el 10 de abril al 24 de mayo del año 2024.

Resultados: se demostró en el metaanálisis que los factores con mayor asociación con los cambios evolutivos de la cardiopatía hipertensiva fueron: la presencia de microalbuminuria (HR: 3,978; IC al 95 %: 2,131-7,428) la proteína C reactiva (HR: 2,141; IC al 95 %: 1,247-3,675) y los efectos propios de la hipertensión arterial (HR: 2,105; IC al 95 %: 1,647-2,692). Los modelos para evaluar el riesgo cardiovascular no mostraron una calibración y discriminación útil para pronosticar los cambios evolutivos de la cardiopatía hipertensiva.

Conclusiones: se demostró la asociación significativa entre los diferentes factores pronósticos y los cambios evolutivos de la cardiopatía hipertensiva. Es evidente la necesidad de una nueva herramienta pronóstica a partir de los mencionados factores en aras de mejorar la exactitud y precisión del mencionado riesgo.
